자폐스펙트럼, ADHD, 그리고 틱장애를 가진 아동의 실행기능 비교 연구 (Comparison of Executive Functioning in Children with Autism Spectrum Disorder, ADHD and Tic Disorder)

초록

본 연구에서는 자폐스펙트럼, ADHD, 그리고 틱장애를 가진 아동들의 실행기능을 4가지 하위 영역별로 그 특성을 비교하였다. 이를 위해 복합형 ADHD아동들 16명, 자폐스펙트럼장애 아동들 15명, 그리고 틱장애로 진단된 16명을 대상으로 일원변량분산분석을 실시하였다. 연구 결과, 주의통제능력과 관련하여 억제능력에서는 세 집단 간의 유의미한 차이는 발견되지 않았다. 둘째, 인지적 융통성과 주의분할능력 능력에서는 ADHD집단이 틱장애 집단보다 저조한 수행을 보이며 두 집단 간에 유의미한 차이를 보였다. 인지적 융통성과 피드백에 반응하는 능력에서 자폐스펙트럼장애 집단이 틱장애 집단보다 저조한 수행을 보이며 두 집단 간에 유의미한 차이를 보였다. 셋째, 정신운동 속도와 순차적 정보처리능력에서는 세 집단 간에 통계적으로 유의미한 차이는 보이지 않았다. 마지막으로 계획 및 조직화 능력에서도 세 집단 간에 유의미한 차이는 나타나지 않았다. 따라서 ADHD아동들과 자폐스펙트럼장애 아동들은 틱장애 아동들과 비교하여 인지적 융통성 및 피드백에 반응하는 능력이 저조함을 확인할 수 있었다.

영어초록

The purpose of this study was to evaluate and compare the executive functions among ASD, ADHD and TD groups. The group differences in neuropsychological test was examined by one-way ANOVA followed by Scheffe post hoc tests. The results of this study were as follows. First, there were no significant differences in the STROOP TEST among the three groups. Second, there were significant differences for ADHD and TD group in the CCTT-2 test. ADHD group showed poorer performance than TD group. Additionally, there were significant differences for TD and ASD group in the WCST test. TD group had the highest performance score while ASD group had the lowest performance score in Total Error, Perseverative Errors, and Conceptual Level Responses. Third, there were no significant differences in the CCTT-1. Fourth, there were no significant differences among groups in the block design subtest of KEDI-WISC. Outcome of study indicated that children with ADHD have poorer performance in cognitive flexibility and attentional set-shifting compare to children with TD. And children with ASD have poorer performance in cognitive set shifting, flexibility in thinking compare to children with TD. However, there were no significant differences in inhibition, planning and psychomotor speed among three groups.
