중장년층의 사회적 배제, 우울 그리고 자살생각의 관계 (The Relationship among the Social Exclusion, Depression, and Suicidal Ideation in the Middle - Aged Individuals)

초록

한국의 자살률은 지속적으로 증가하고 있음에도 불구하고 이에 대한 자살예방정책은 자살을 개인심리적 차원에서 바라보는 ‘자살의 개인화’에 국한되어 있다. 본 연구는 자살에영향을 미치는 배경에는 사회적 배제와 같이 다차원적이고 복잡한 거시적 원인이 존재하고 있을 것이라는 ‘자살의 사회화’ 가정하에, 중장년층의 사회적 배제, 우울, 그리고 자살생각의 관계를 살펴보았다. 이를 위해 한국복지패널(KOWEPS) 제 9차년도(2014년) 자료를 활용하여, 사회적 배제가 중장년층의 자살생각을 설명하는 직접경로와 사회적 배제가 우울을 매개하여 자살생각에 도달하는 간접경로를 통계적으로 검증하였다. 분석결과, 중장년층의 사회적 배제와 자살생각 간의 관계에서 우울이 가지는 매개효과를 검증하였으며, 경제활동 배제, 건강 배제는 우울을 통해 자살생각에 이르는 경로를 완전매개하는 것으로 나타났고, 노동활동 배제, 주거 배제, 사회참여 배제는 우울을 통해 자살생각에 이르는 경로를 부분매개하는 것으로 나타났다. 이러한 연구결과를 바탕으로, ‘자살’ 의 원인을 개인의 정신건강 문제로 접근하는 정신의학적 접근만이 아닌 사회구조적 측면을 포괄적으로 반영하는 사회적 배제 차원의 접근을 시도하여 중장년층의 자살개입 정책관점 전환에 대한 필요성을 제기한다.

영어초록

The suicide prevention policies are limited to the ‘individualization of suicide’ which looks the suicide in the personal psychological dimension even though the suicide rate in South Korea is constantly increasing. The study examined the relationship between the social exclusion, depression and suicidal ideation of middle-aged individuals under the premise that there will be multidimensional and complex macroscopic causes like the social exclusion on the background that influences suicide. For this, utilized the Ninth(2014) data from KOWEPS, the study statistically verified the direct route where the social exclusion explains the suicidal ideation of middle-aged individuals and indirect route where the social exclusion reaches the suicidal ideation by mediating the depression. The results show that, exclusion of economic activities and health completely mediated the route to suicidal ideation through depression. Moreover, it was revealed that the exclusion of labor activities, housing, and social participation werepartially mediating the route to suicidal ideation through depression. The findings suggest that the necessity on the shift of perspective on the suicide intervention policy by trying out not only the psychiatric approach which approaches the cause of ‘suicide’ with personal psychiatric problem but also the approach in the dimension of social exclusion which comprehensively reflects the socio-structural aspects.
