망막중심정맥폐쇄에서 방사상시신경절개 후 망막맥락막 곁순환의 형성 (Retinal Choroidal Collateral Circulation After Radial Optic Neurotomy for Central Retinal Vein Occlusion)

초록

목적 : 방사상시신경절개술을 시행한 망막중심정맥폐쇄 환자에서 망막맥락막 곁순환의 형성 빈도와 곁순환의 형성과 시력 변화와의 관계를 알아보고자 하였다.
대상과 방법 : 망막중심정맥폐쇄로 진단 받고 유리체 절제술 후 방사상시신경절개술을 시행받은 환자 17명 17안을 대상으로 의무기록을 후향적으로 분석하였다. 수술 전 후 최대교정시력 및 안저검사, 형광안저촬영 등을 실시하여 초진시의 최대교정 시력에 따른 망막맥락막 곁순환의 형성 빈도를 알아보고, 곁순환의 형성에 따른 최대교정시력의 변화를 비교하여 알아보았다.
결과 : 방사상절개부위에 망막맥락막 곁순환이 형성된 경우가 17안 중에서 9안(52.9%)에서 발견되었으며, 초진시의 시력이 0.02 이상인 그룹(72.7%)에서 초진시의 시력이 0.02 미만인 그룹(16.7%)보다 곁순환이 형성 되는 빈도가 높았다(P=0.043). 또한 수술 후 시력의 호전과 망막맥락막 곁순환의 형성은 통계적으로 유의하였다(P=0.008).
결론 : 초진시의 시력이 0.02보다 좋은 군에서 망막맥락막 곁순환의 생성 빈도가 높았으며, 망막맥락막 곁순환의 형성이 수술 후의 시력 호전에 도움을 주는 것으로 생각된다. 또한 앞으로 많은 증례에서 질환의 자연 경과와 비교연구가 필요하리라 사료된다.

영어초록

Purpose: To evaluate the incidence of retinal choroidal collateral circulation after radial optic neurotomy (RON) with central retinal vein occlusion (CRVO) patients and to correlate these collaterals with changes in visual acuity.
Methods: We conducted a retrospective study of 17 eyes of 17 consecutive patients diagnosed with CRVO who underwent RON after a standard three port-vitrectomy. Fundus examination and, FAG were performed to evaluate the incidence of retinal choroidal collateral circulation according to preoperative best corrected visual acuity. We evaluated changes in best corrected visual acuity according to chorioretinal circulation formation.
Results: Retinochoroidal shunts developed in 9 eyes (52.9%) at the site of radial optic neurotomy. The group whose initial visual acuity was better than 0.02 (72.7%) developed more shunts than the group whose initial visual acuity was under 0.02 (16.7%) (P=0.043). Changes in visual acuity were highly correlated with the development of collaterals from the retinal to choroidal circulation (P=0.008).
Conclusions: Patients whose that initial visual acuity is better than 0.02 have more retinal choroidal collaterals. Surgical induction of retinochoroidal venous anastomosis may result in visual acuity improvement. Randomized studies are needed to compare the current study modality with the natural course of central retinal vein occlusion.
