노인요양시설 돌봄의 질 패러다임 경로분석: 구조-과정-결과 인과사슬 (Nursing Homes to be Paradigms of Path Analysis: A Causal Chain of Structure, Process and Outcome)

초록

본 연구는 노인요양시설 돌봄의 질 패러다임 경로를 확인하기 위해 Donabedian의 이론적인 방법론인 구조, 과정, 결과 차원을 적용하여 연구모형을 구성하고 실증적인 분석을 수행한 연구이다. 경로분석을 통하여 3개 차원의 7개 요인이 분석되었으며, 모형의 신뢰도와 유의성 및 적합도가 검증되었다. 특히 구조의 질-과정의 질-결과의 질 경로 패러다임의 인과성 및 연계성이 완전매개 되는 노인요양시설 인과사슬(Nursing Homes Causal Chain)을 확인하였으며 그 효용성을 검증하였다. 노인요양시설 인과사슬을 통해 노인요양시설이 첫째, 지속적인 질 향상을 목표로 정보를 수집하고 처리하기위한 절차와, 둘째, 질 개선활동의 실제 설계 및 구현을 위한 토대를 마련하고, 셋째, 돌봄의 구조-과정-결과 차원 사이의 관계의 방향을 재인식하고, 노인요양시설 돌봄의 질 개선 전략의 기초적이고 유용한 도구로 그 활용성을 입증하였다.

영어초록

In this paper presents a theoretical methodology of the Donabedian's paradigm for path analysis. Therefore, Structure, Process, Outcome research dimension by configuring the model and the empirical analysis of studies performed in nursing home care quality. Path of the seven factors were analyzed by three-dimensional analysis, reliability and significance and goodness of fit of the model was verified. In particular, the Structure Quality-Process Quality-Outcome Quality path causality and linkage is complete mediated, Nursing Homes Causal Chain was developed to verify its effectiveness. Through Causal Chain, First, the procedure for collecting and processing the information targeting the continuous quality improvement, secondly, laid the foundation for the actual design and implementation of quality improvement activities, and third, reaffirm the direction of the relationship between structure, process, outcome level of care provided, and proved its usability as a direct and useful tool for quality improvement strategies.
