노인의 우울, 절망감과 자살생각 간의 구조적 관계 (The Structural Relationships among Depression, Hopelessness and Suicidal Ideation in the Elderly)

초록

본 연구는 우리나라 노인의 우울, 절망감과 자살생각 간의 구조적 관계를 검증하기 위해 수행되었다. 연구를 위해 D광역시와 K도에 거주하는 60세 이상의 노인 271명을 대상으로 우울 척도, 절망감 척도, 자살생각 척도 질문지를 사용하여 자료를 수집하였다. 본 연구에서는 선행연구를 근거로 우울을 매개변인으로 하는 완전매개모형과 부분매개모형, 그리고 절망감을 매개변인으로 하는 완전매개모형과 부분매개모형을 상정하여 변인들 간의 관계를 가장 잘 설명하는 모형을 탐색하였다. 자료 분석을 위해 SPSS 18.0과 AMOS 18.0 프로그램으로 구조방정식 모형 분석이 실시되었다. 본 연구에서 상정한 모형들에 대하여 적합도과 구조경로의 유효성을 비교 검증한 결과, 절망감이 우울을 매개로 자살생각에 영향을 미치는 우울 완전매개모형이 가장 적합한 모형으로 채택되었다. 이때 절망감과 자살생각의 관계에서 우울의 매개효과는 유의한 것으로 검증되었다. 본 연구에서 나타난 결과는 우리나라 노인의 자살생각과 관련된 우울과 절망감의 발달 경로와 상대적 기여도를 규명한데 의의가 있으며, 노인의 자살생각 예방을 위한 조기 개입 방안과 차별화된 전략의 필요성을 시사한다.

영어초록

This study aimed at examining the structural relationship in which depression and hopelessness affected suicidal ideation among the elderly. A total of 271 senior citizens (over the age of 60) inhabiting D city and K province participated in the present study. The measures included the Geriatric Depression Scale, Beck Hopelessness Scale, and the Scale for Suicide Ideation. Based on previous research, a full/partial mediator model with depression as a mediating variable and the full/partial mediator model with hopelessness as a mediating variable were constructed. Hypothesized models were tested respectively in order to investigate which model best explained the relationship among the three variables. The collected data was analyzed with SPSS 18.0 and AMOS 18.0 and structure equation model analysis was then conducted. When the validity of the pathway was analyzed, the model with depression as a full mediator was adopted. Therefore, the significance of depression as a mediating variable between hopelessness and suicidal ideation was relevant. The significance of this study lies in the fact that it validates the pathway in which depression and hopelessness develop and affect suicidal ideation. The results suggest that early intervention and the specialized treatment for depression are necessary in preventing suicidal ideation amongst the elderly.
